Advance Care Planning

By making plans that cover our care, lifestyle, health and finances, we work out what we want, as well as making things easier for those around us.

Our team offers two hour Introduction to Advance Care Planning sessions. These sessions are modelled on the community workshops developed by Palliative Care WA.

Upcoming sessions:

We also support people to complete their Advance Health Directive which is a legal document that helps family, carers and health professionals understand what is important in terms of a person’s health and health care. Over the course of two, two hour sessions which are held a fortnight apart, we have found most people are able to complete their Advance Health Directive ready to share with the people who matter.

Upcoming sessions at the BUNBURY LIBRARY:

Tuesday, 31 March & 14 April | 1.30-3.30pm

Thursday, 16 & 30 April | 1.30-3.30pm

Thursday, 14 May & 28 May | 1.30-3.30pm

Thursday, 4 & 18 June | 1.30-3.30pm
